Longitudinal Case Management for Refugees: Toolkit
PRHL’s longitudinal case management program supports newly arrived refugees for 6-12 months. Through regular meetings with student case teams, refugees learn the basics of the US healthcare system, formulate actionable goals to better manage their health, develop skills to better manage their overall health, and are empowered to confidently navigate the healthcare system. Our toolkit includes the key resources needed to implement our unique program for your target population.
